Capitation payment for gynaecologists to improve access and preventive care

Starting in 2026, Czech health insurance funds will introduce a new capitation-based payment model for outpatient gynaecological care, making gynaecology the last segment of primary care to adopt this reimbursement method. Under the new agreement between insurers and gynaecologists, providers will receive a regular monthly payment for each registered patient – conditional on the patient attending at least one preventive examination in the past two years. The aim is to improve access to care, strengthen prevention and incentivise gynaecologists to expand patient registration. The capitation will cover routine care, including standard clinical and ultrasound services, while specific procedures (for example, pregnancy care, infertility treatment, uro-gynaecology) will remain reimbursed on a fee-for-service basis. Bonuses will be available for practices that meet criteria such as extended office hours or continuous medical education. The new scheme is pending confirmation through the Ministry of Health’s annual reimbursement decree.
